The situation

What has happened

Aisha Rahimi is 27, receives daily living supports morning and evening, uses a hoist with two workers and has epilepsy. Her family speak Dari and her plan nominee has complained that the last plan review went ahead without an interpreter, so the family felt decisions were made without Aisha. Her plan review is due on 28 September. Aisha has told workers she wants to spend more time in the garden and would like to attend a women's swimming session at the local pool. Her NDIS plan includes community access hours that are not being used. Anjali Deshpande wants this review to be a model of how Willow does it.

Your brief. Prepare and run Aisha's plan review with an interpreter, build a support plan around her own goals and her NDIS plan, make sure her safety needs are documented for every worker, and close out the complaint so the family can see what changed.

What to look for

Evidence guide

Look for a plan written in Aisha's words with goals that link to her NDIS plan, evidence the interpreter was arranged before the meeting and that Aisha, not the nominee, led the decisions. Safety information must be complete enough for a new worker to act on. The complaint response must state what changed, not only apologise.
