The situation

What has happened

Tayla Nguyen is 16 and has been coming to the drop-in for a month. She has been staying with a friend's family for two weeks and told Jarrah Williams that they cannot keep her past Friday. She does not want to return to her mother's home and has not said why. A youth housing referral was lodged last week but no one has called her back. Jarrah is on the outreach van and has asked you to take the lead. Tayla is anxious about being reported and has asked you not to tell anyone. It is Thursday afternoon and the weekend is close.

Your brief. Engage Tayla respectfully, complete a safety plan with her for the coming weekend, escalate the housing referral, decide what you must report and to whom, and record everything accurately in the progress notes, while being honest with Tayla about the limits of confidentiality.

What to look for

Evidence guide

Look for an honest explanation of confidentiality given before Tayla discloses more, a safety plan in Tayla's own words that covers the weekend, and a reasoned decision on reporting that refers to the policy rather than a guess. Progress notes must separate what Tayla said from what the student thinks. Check the student consulted the manager rather than deciding alone.

The student's evidence summary lists every record they created or changed in the systems named above, their notes and the tasks they ticked. Verify it against the deliverables and your own assessment tool.
