Review a wellness plan with the person who owns it
You are a Recovery Support Worker in the Mental Health and Wellbeing team.
What has happened
Ben Carmody is 38 and has received psychosocial support from Willow for eight months. He has a wellness plan and a crisis plan, a GP mental health treatment plan, and attends the Friday peer group. Last week he had a difficult time, missed the group and phoned Sam Okafor to talk through his early warning signs. He did not express any safety concerns. Ben has also asked to become a peer volunteer. Ruth Abela has asked you to visit Ben this week, review his plans with him, check in with his GP with his consent and start a conversation about the peer volunteer pathway.

The master record of every person Willow supports, their program, coordinator, plan review date, risk flags and service status.

Evidence guide
Look for a plan in Ben's own words that he agreed to, evidence the student asked for and recorded consent before contacting the GP, and a GP summary limited to the consented content. The progress note must be strengths based and objective. The escalation note must name the on-call line, the manager and the crisis services in the right order.
