Home and Community Care
Delivers in-home and community supports for older people funded under the Support at Home program and the Commonwealth Home Support Programme. The team assesses needs, builds care plans with the person and their family, rosters support workers and monitors changes in health, mobility and safety at home.
What the team is accountable for
- Intake, assessment and care planning for older clients
- Personal care, domestic assistance and social support visits
- Falls, dementia and medication risk monitoring
- Liaison with GPs, allied health and hospital discharge teams
- Care plan reviews and Support at Home budget tracking

The master record of every person Willow supports, their program, coordinator, plan review date, risk flags and service status.
Roster and Shift Log
The seven-day roster of client visits and shifts, with check-in status, hours worked and shift notes for the rostering desk.
Progress Notes and Case Log
The contact record for every client: shift notes, phone contacts, case conferences and handovers, written objectively and signed off by the coordinator.
